So first of all I think people can do whatever the fuck they want as long as A) The know the risks and B) They are not putting others at harm. With that said I think in many instances of BBFS the above is not met.
A) Risk. The risk of bacterial infections such as gonorrhea, chlamydia, syphilis is quite high with BBFS with an "unknown" host in the SW, I think everyone knows that and I will not get into it. Risk of Hepatitis is also high.... What I have noticed is that people really care about the big one, HIV even though the other infections are quite debilitating. Also I have seen people downplay the risk of HIV... Again risk is relative and the facts are not well known. There is a systematic review that has high level evidence on this (source). I have seen people cite these numbers as being "low". People fail to realize though that the per event risks are based on the assumptions that the hosts of the infection are likely with known chronic HIV and on treatment. Some estimate if the host has an acute infection that has yet to be diagnosed the risk of spread can be 60-80 times higher since they have high circulating viral loads.... And the latter is most likely the case if one encounters this via this hobby. It takes one infected host with this high viral load and a clientele of guys wanting BBFS to cause a serious outbreak. And once there is an outbreak that is identified by public health authorities stemming from escorts you sure bet that the authorities are gonna have ammunition to come after everyone legally and they may be justified... Again someone people may be told that the risk of them getting infections is 90% and they will still do it... thats their choice. But if your doing this educate your self on the risks and think with your head not your dick....
I personally think the risk of this is so high for me that I am prepared that in an event of a "Failed condomn" on a provider that I do not know or I suspect may partake in BBFS I will go to the emergency or walkin and get a prescription for post exposure prophylaxis for HIV and test my self for Hep B C and bacterial STIs. This has to be done in 72 hours. I actually think that while there is no consensus on this the expert opinion of ID doctors may be in favor of prescribing PEP for those exposed to insertive anal or vaginal sex with an unknown partner that is in sex work providing BBFS. IM raising the last point as that may mean the drugs will be covered, otherwise they are expensive meds. So if you have an accident with a provider that you suspect is doing BBFS go see a doctor within 72 hours, that is if you care and the risks are relevant to you. Alternatively stick to safe providers you know, I am personally adopting this as my health is #1 priority.
B) Harm
There is no doubt that providers can be impacted by mental health, this happens to anyone in any line of work.
