Actually, the Health card is used to help fill in data on Medical forms for Hospitals and Doctors. Considering they need to know your biological Sex, the fact that is stays binary seems appropriate. When a doctor is trying to diagnose issues in your abdomen, they don't give a shit what you identify as. They care what sex you actually are.

Pretty sure that even an online degree doctor would be able to determine whether there is a uterus or not.
