I've never heard that you can die trying to have sex with a flaccid penis. Every put it in and watch it grow inside the lady (be it mouth or moist vagina)?
(His dad could've had a heart condition, or was on drugs, etc.).
I'm in my 50s. Recently, before a prebook, I had a headache that made me think about cancelling. I had been nursing this low level headache for most of the day, probably stress related. During the sex that evening, the headache came back a little more intense this time. For the first time ever I was concerned and slowly ended the session without climax. Warning or just stress? Although it may be someone's dream, it's not the way I want to leave the world. Still had the headache later at home.

Good for you to play it safe though. I had very intense sex in a warm room and felt a momentary chest pain. Slowed it down, and took a cold shower. Was able to finish though. Was feeling off later, so I drove to the hospital and had myself checked. EKG and blood test were negative for any unhealthy cardio conditions. It was a combination of hot room, intense play and my worrying self playing mind games on me, which might have caused a panic attack too.
My point was that having sex with a flaccid penis was likely
the physical condition of the father at the time of death. Didn't
mean to say people can die that way. Stuffing a soft penis into
a lady could be done and was far from a strenous exercise than
shoveling snow in cold weather. If the father already had a heart
problem he was unlikely to be able to have great sex in the first place.
I'd rather die living a healthy life in my 80s and 90s even if it was
sexless near the end than while having lousy sex in my late 50s.
