This actually happened to me once, but without the scary aftermath. Didn't even know the condom broke until I went to take it off. This was several months ago, I have heard from her but nothing came of it, she told her agency about it immediately and took the necessary precautions. Needless to say, I did go for tests afterward.

There is of course the other meaning to the phrase "knock her up". In the NW or over'ome when you knock someone up you in fact wake them in the morning. So I suppose if you had an all night outcall you could very safely knock her up in the morning.
