I have never been charged more for a bbbj. However if the escort asks if a cbj is ok I allow it, for her peace of mind, but that is unusual.

Never had an SP charging me more for BBBJ. If that would happen I would be out the door.

That said I have respected SP telling me it would be CBJ. I just don't repeat.

In Canada, uncovered is a standard service far more often than not.
