As to why the truck failed to stop, that's what the crash investigation will try to determine. In any event, it was likely either mechanical failure, impaired driving, distracted driving or he fell asleep.
The crash only happened 10 days ago. They have to sift through a large amount of debris looking for answers, and that takes time. The event data recorder (black box) is only one aspect. REGINA—Saskatchewan RCMP have charged the 29-year-old truck driver in the fatal bus crash that involved the Humboldt Broncos junior hockey team.
Mounties say the arrest was made Friday morning in a collision between the bus and a semi-trailer at a rural Saskatchewan intersection on April 6.

Jaskirat Singh Sidhu has been charged with 16 counts of dangerous operation of a motor vehicle causing death and 13 counts of dangerous operation of a motor vehicle causing bodily harm.
The Broncos were on their way to a playoff game when the crash occurred, killing 16 people and injuring 13 others. Most of the injured players have been released from hospital.
The driver of the semi-trailer was not hurt. He was taken into custody immediately after the collision and released later that evening.The driver was working for Adesh Deol Trucking Ltd., a two-truck company operating out of a home in northeast Calgary.
The company said the driver of its oversize tractor-trailer only had a year’s experience as a commercial driver and just over two weeks of training from his new employer.
“I do my best. I give the training within 15 days,” Sukhinder Singh, owner of Adesh Deol Trucking Ltd., told reporters back in April.
“I can train for the straps, secure the load — everything. But the guy has already a licence one year ago.”
Singh may have been confident of his employee’s readiness, but the country’s trucking lobby reacted with surprise to news that such a novice was operating a two-trailer vehicle — a configuration known in the industry as a B-train — on wintry Prairie roads after just a month on the job.
In June, RCMP said they were preparing to talk to Crown prosecutors about potential charges.
Officers were still investigating at that time and had only said that the semi-trailer was in the intersection when the crash occurred.
“We need the evidence and facts first, and that’s what is ... adding to the extra time here to work through that process,” Williams said in June.
The devastating bus crash rocked the country, prompting a massive show of sympathy across Canada, including homeowners leaving hockey sticks on their front porch and workers wearing sports jerseys to the office.

The Charges are not surprising albeit tragic when the truck driver ran a red light stop.

The investigation took 3 months to complete. Given the severity of the crash, it's not surprising it took so long. Supt. 16 charges of criminal negligence causing death and 13 causing injury. Not sure why it took four months to come up with the charges.
The offences of dangerous operation of a motor vehicle causing bodily harm or death carry maximum sentences of 10 years and 14 years, respectively.Doubt it unfortunately.
