Maple Leaf Gardens.

I went to one of those games where Phil played the big, bad Bruins, of which Eddie Shack was now a member. He got into a vicious stick incident with Flyer Larry Zeidel who was Jewish. Evidently the Bruins were hurling racial epithets at him until he'd had enough. He and Shack carved each others' faces with the tips of their sticks. Ugly incident.
MLG is correct, but they also played in 2 other arenas.

Who is the only major league baseball player to hit a ball that resulted in a human fatality?

He was 31 years old when he was selected by the Montreal Expos with the first pick in the 1969 National League Expansion Draft. His last major league hit was at age 42, in a season in which his batting average was .429. He had one hitless at bat two seasons later, and still works as a coach for the team to which the Expos traded him.

Manny Mota
Correct!

On May 16, 1970, Mota hit the only batted ball in major league history to cause a fatality. In the bottom of the third against the Giants at Dodger Stadium, Mota fouled one off of Gaylord Perry along the first base line. The ball struck 14-year-old Alan Fish in the left temple. Four days later, Fish died of an inoperable head injury. (from Wikipedia)
