I think it is a prerequisite for driving manual. There are no options. If you can't shift, then you can't drive manual.

As to the degree of difficulty, I imagine hundreds of millions, if not billions, of people have mastered it. So it is obviously not difficult for anyone remotely co-ordinated and intelligent.​
