I talk to them like we are old friends, but when it comes to what happens next, I let them take the lead until they are comfortable. Lots of questions, "is this okay? can we do this?"
Don't think of it as following a script, because if she doesn't follow along, you'll stumble and lose track of what to do "next" - just talk to her like she is a person (shocking, I know)

For me, it was usually something like: "Hi (insert SP's name here)... I'm AJ, its so nice to finally meet you!" I always try to use the lady's name. Its more personal.
Depending on the situation, this is usually greeted with a warm embrace and sometimes a kiss, which usually morphs into some LFK, DFK. We then make some small talk, and I usually compliment her on her outfit, her shoes, or her general appearance, and then excuse myself to freshen up.
With one exception, I have always established a rapport with the ladies I see, though e-mail and/ or text, so that awkward first moment never happens. Its one of the reasons I prefer to see indies.

For me, it's when he says "Ohhhhhhhhhhh" when he first sees me. Definitely sets the stage for a hot encounter.
