A few tips ...
I just got a new passport made 2 months ago and the procedure is very simple. Once you pick up the application form, there are explanation's given on the form itself regarding how the pictures are to be taken, what references you need, and who can sign the declaration. For the pictures, just go to a photo studio and they know what is required .. they stamp the photo's (you need 2) with the date so that the decalarator can sign it. The declarator can be you doctor, optimeterist etc ... it tells you on the form. I would suggest calling them up as soon as you get the pics taken and the forms so you don't have to wait too long for an appointment.
For ID, I just took my driver's license, SIN card, HLTH card, and citizenship card ... I suggest you take all of them in case they require further id. All they do at the passport office is have a look and hand it back to you.
Try booking a day off work because it's hard to know what the wait will be like. Try going early in the morning and getting a line number. They will tell you the time-frame within which your number will be called. If the office opens at 9am, try going there an hour early so you can beat the rush ... that place is always busy.
The fee for a new passport is $85 and they will express mail it to you within 10 business day's. If there is an emergency, you can tell them you need the passport early ... might be an extra fee.
Double check the form after you fill out and make sure the required signatures are there. Otherwise they might send you back and you will have to wait in line all over again.
