I would actually recommend not going to the sexual health clinic in this specific case. Simply because the issues he’s presenting with are widespread and not localized to his genitalia. It’s unlikely it is an STI. But even if there is that chance, he’ll get more comprehensive care going straight to his GP for all the symptoms he’s presenting with.I know a bit about medicine.

The back rash does not sound like much of anything to worry about. The only one would be syphilis, which clears after. a week or so.
The penile lesions could be herpes. This can DEFINITELY be pass without penetration. HSV is a very infectious virus and all you need is skin to skin contact. No way to know without seeing it, though.
While the sexual health clinic is a great resource, they don’t actually test for HSV and for some conditions they may just refer you back to your GP because it’s beyond their scope of practice.
If you want to avoid multiple trips to a doctor, just go straight to your GP or a walk-in clinic.
