I know a bit about medicine.

First, see a doc. In Ottawa, can go to the sexual health clinic on Clarence.

The back rash does not sound like much of anything to worry about. The only one would be syphilis, which clears after. a week or so.

The penile lesions could be herpes. This can DEFINITELY be pass without penetration. HSV is a very infectious virus and all you need is skin to skin contact. No way to know without seeing it, though.
I would actually recommend not going to the sexual health clinic in this specific case. Simply because the issues he’s presenting with are widespread and not localized to his genitalia. It’s unlikely it is an STI. But even if there is that chance, he’ll get more comprehensive care going straight to his GP for all the symptoms he’s presenting with.
While the sexual health clinic is a great resource, they don’t actually test for HSV and for some conditions they may just refer you back to your GP because it’s beyond their scope of practice.
If you want to avoid multiple trips to a doctor, just go straight to your GP or a walk-in clinic.

When you say white blisters how do they look?
Are the ones on the penis the same as the ones on the back?
If so it is likely it could be molluscum.
Molluscum can spread from dirty towels or even a hot tub.
Usually kids get it but am wondering if you used an infected towel on your back and genital area.
In any case go to your GP.
If it is molluscum they will use liquid nitrogen to burn it off.
