I'm not sure anyone's given you a thorough answer yet. Let me take a shot at it. My dudes, feel free to correct or add anything if you'd like.
Just to preface before we talk protocol, if you're looking to stick to Durham then PA and Lavilla are basically your only options. There's another one somewhere around PA that you'll see advertising on BP and CL but it's gotten meh reviews and seems pretty low quality just from taking from their quality of ads, no website, and lack of consistency between MPs.
Anyway, here's what you should know...
1. Read reviews before you go and pick someone who's well reviewed. You don't want you first experience to be a bad one because then you'll never go back. Based on my own experience and on 100 other reviews, I'd recommend Sydney. She's young, pretty, friendly, genuine, and gives you a hell of a time. Other people have stronger likes of other girls, but I think you'd find Sydney to be the most well rounded. Check the website for the schedule and call ahead to book.
2. Know the session you're looking for. Topless, nude, nude reverse (for a time, you get to massage her), or VIP/body slide (the sexiest of sessions, she'll slide her body all over you and, depending on the girl, you'll get things like Russian, simulated riding, hot dogging, etc. But that's YMMV depending on girl, body type, and how much she likes you). Know that if you go for the topless or the nude, many MPs will spend most of the session trying to up sell you because they're getting paid significantly less than they would for the Nude Reverse or the VIP.
3. At PA and Lavilla, you can pay with cash or with debit or credit. With cash, they request a door fee up front ($40, I think...?) and then you give the rest to your MP depending on the session type. If you pay with debit or credit, they charge you the full amount (door fee and session type) after the session is done. You can still tip your MP in room with cash though if you'd like
4. Tipping. A matter of hot debate is whether you should tip or not. I'm in the NOT camp, but if you find yourself a regular girl and tip her every time you see her, she's likely eventually going to open some doors for you that others never get to see behind. Again, that's YMMV. Your call. Standard would be... what... $10-20?
5. Always say yes to a shower. It's just good manners and will put your MP at ease. After your shower, don't get dressed. I prefer to wear my towel around my waist so that I can stand up to greet the girl when she enters without shocking her by hanging brain right as she enters the door. It won't be long after the greeting that she asks you to drop the towel and lie face down on the table.
6. Manners once on the table are somewhat up to you. Other than the obvious of being respectful and nice, guys play out the session in different ways. I'm a shy guy who doesn't want to come on as too aggressive or assuming, so I usually let the MP lead the whole session. I think I probably miss out on a lot because I don't ask enough questions and don't let the MP know what I like and don't like. Other guys are great at this.
7. If you've asked for the NR or VIP, your MP is going to be ok with you touching skin and tits through the session. If they're not ok with you touching their tits, then you've gotten the wrong MP. Good MPs will make sure you're touching them everywhere as much as possible. Whether or not you're allowed to touch their pussy with fingers and/or tongue is always a bit of a mystery. Some girls are ok with it, some girls aren't. Best thing to do is just ask - they're used to it.
8. I'm not sure if this goes without saying so I'm going to say it - don't be a dick and don't be dirty. If you're demanding and rude or if you smell like shit, your MP is going to give you a $5 session or just kick you out if you're really bad.
