If the shower is ensuite to the bedroom, I walk out naked, and exclaim "Ta-Da!", even with my wrinkly old grandpa bod. 
Same if the unit has only one escort working there, even if the shower is in a separate bathroom.
If another escort is working in the unit, and If the shower is in a bathroom separate from the bedroom, I walk out of the bathroom and enter the bedroom wearing a towel.

. For me the answer is "all of the above", depending on the circumstances and who I'm seeing:

The whole point of showering immediately before you hop into bed is to be fresh as a daisy and odour free. Putting clothes back on, even just the underwear, is a step backward.

I used to put my underwear back on but I discovered that this is considered to be bad etiquette and unsanitary so I've just started coming out in a towel.
I book an extended session with one of my regulars and I bring a fresh pair of underwear for the drinks, snacks and YouTube videos part of our time together.

Been told this by a few providers - the very worst option is to put underwear back on! (unless you can "prove" you are wearing fresh ones etc.)

For me if we were both clothed before I left to take a shower, I come back out dressed. If I had her in some manner of disrobement, I might just wear pants (or Winnie the Pooh-it lmao). If she's fully nude, I come out in the birthday suit.
Towel exit is only a first-time-with-a-provider move, IMO.
