Actually, I think it's just the result of there being a larger population base, therefore resulting in more variety amongst personality types. In terms of fetish players, the most active North American cities in no particular order are NYC, San Fran, LA, Chicago and Toronto.

My personal experience has been that fetishists and BDSM afficionados tend to be of high intelligence, with careers in high stress levels jobs which place them in positions of responsibility. Larger cities, of course, have more people who fit this profile.
