Mine was in 2001. I used to post regularly on the message board for the game show The Mole, (sort of an intellectual version of Survivor, and the source of my bobistheowl handle). At one point, I was talking about "the game, within the game, within the game", and I expressed that in mathematical form as:

{ga[ga(game)me]me}

A whole lot of people on the message board misinterpreted that, and thought I was saying that I was gay.

About 15 years ago, I used an internet cafe that had a plastic half bubble above the seat which apparently enhanced the sound. I was watching the Pamela and Tommy Lee video when the sound unexpectedly came on loudly while the cafe was quite full.

I struggled to mute the sound and skulked out quite red faced!

I used to teach Information Technology at a career college (6-month-fast-track-to-a-new-career kind of thing).
During labs, I would regularly turn off the projector and surf TERB on my instructor PC.
Well, once I forgot to turn it off... not really sure how long I was broadcasting TERB on the wall before I clued in... I'm POSITIVE at least a couple of students saw, although no one ever actually said anything (to me anyways).
