His whole shtick was to say the most outrageous shit to get a reaction out of those on the left and then dig even deeper. He said stupid shit about Muslims, feminists, gays, etc. etc. The alt right crowd loved him because he was gay and finding gays to say stupid alt right shit was not easy. Milo parlayed the whole thing into a shit load of money and fame. Well, fame for 15 minutes at least.
And then he crossed a line when he spoke approvingly of gay men having with young boys as if it was OK. I don't know what the fuck he was thinking there. He's been kryptonite ever since and no-one wants anything to do with him. He lost his book deal, his job at Breitbart and all the invitations to say outrageous shit. And good.
Now he's back at his old bag of tricks saying stupid shit to get a rise out of those on the left and hopefully get his old life back. But I really doubt its going to work. His buddies on the right never really liked him, and only tolerated him so they could point to him as their gay friend.
