Correctly identifying the biological gender of intersexed infants & children is an evolving art. Even today, pediatricians & medical doctors are often spoofed. She's 41, so the doctors dealing with her would have been trained & socialized in the 1950's

Sometimes the issue would become more clear if it were left alone for a few years, but because of social pressure to identify the child and socialize it in a gender specific way, a hasty decision is often made to brand the kid one gender or the other without letting the delayed development play out.
Without seeing the equipment she had before the surgery, we can't make a good guess as to weather it was a legit penis or a
Clitoromegaly. The article also doesn't reveal if she had actual and viable testes or prostate. But the fact she always had a Uterus seems to come down on the side of Female as far as the biological identity is concerned, so I believe the surgery was more a resotration than a re-creation.
In this case everyone made the right decision in not rushing to intervene surgically before puberty but instead allowing her to decide at an age when she has some idea of what it's going to mean.
