The whole point of this is to claim - yet again - that COVID only kills "the weak and the sick". They just rule out everyone with any other health concern they consider " serious" .
You have diabetes and therefore are expected to live for a couple more decades but die of COVID? You don't go on the chart because it isn't a real COVID death. Just do that with a bunch of conditions and your death rate plummets to the numbers seen in the chart.

Easy peasy.
