My Note 4 is only 5 months old but GPS issues exist .. Always get message GPS signal lost and my phone app maps issue shows me never moving while I am driving to my destination

Not here...mine works great no issues here.
Try a hard rest. Maybe one of your apps is messing it up. Maybe also try turning off access to the apps that use the GPS.
