Filling out eye exam forms

I have an eye exam tomorrow morning, the optometrist sent me an e-mail with forms to fill out and i'm supposed to e-mail the forms back, I clicked on each box where I write my name, my address for example, my problem is I can click on the box as i want to write something but no letters are being displayed, am i doing something wrong or do i have to download something in order to fill out the form, thanks for the help

I wouldn't worry about it to much.. fill out what you can, they will ask again if they need the info. I went a few weeks ago and could not answer a couple of questions and was told its ok
