The idea is that the gentleman pays for a certain period of time (i.e. 30 mins, 1 hr) and then sit down for a BJ. A lady starts, and then every increment of time (i.e. 10 mins, 20 mins) she leaves and a new lady comes in to keep going. The point is to tease and go slow, so that he makes it to the end, so to speak. There are countless variations on this theme (you pick the girls vs they are a complete surprise; the women overlap for a few minutes vs one leaves and the next comes in; you could be blindfolded so you don't know who did what, etc,).
