Actually, Sex is assigned at conception, depending on whether or not the sperm cell fertilizing the egg cell carries an X chromosome or a Y chromosome. A doctor or midwife doesn't subjectively decide to anoint a newborn as Male or Female, depending upon their mood or their political beliefs.
The external genitalia are physical manifestations of the presence of the X or Y chromosome in the infant's DNA. In most cases, the biological Sex of a newborn is unambiguous. A "person's internal sense of who they are" pertains to gender identification. No qualified medical practitioner is going to say 'Well, I see testicles and a penis, but I won't be able to decide the Sex if this child until after they decide what they want to be'. No amount of wishful thinking can turn one's X chromosomes into Ys.
Essentially, all trans individuals are wishful thinkers. They aspire to be different from what they are. Surgery, makeup, wigs and lipstick can make anyone appear to be something that they are not, but a change of appearance is only that; an illusion, a delusion, or both.

Wishing that one had been created as a woman is not the same as being a women, beyond the boundaries of the fantasy world in which one resides.
Any individual has the right to present themselves socially as they so choose. Society as a whole, however, has no obligation to accept all personal presentations at face value.
Transwomen don't wish they had been created as a woman. They identify as one.

There are any of a number of reasons why a man may chose to identify and present himself as a woman which have nothing whatsoever to do with a personal journey of discovery. Here are a few:
They are 'the new guy' in prison, and they are unable to protect themselves.
They want to be exempt from military conscription.
They are passengers on a sinking boat which has insufficient lifeboats for all.
They don't want to have to pay for their drinks during Happy Hour.
They want to beat girls in sporting events.
They are homosexual men who are attracted to heterosexual men, so they use deception to entice the 'new', gullible and/ or intoxicated to eat of the 'forbidden fruit'.
Sounds like you have a lot of experience talking with trans men about their feelings.
If that is true, then, most people are wrong as they have been about a number of things in the past.
That is changing and there will be a time, not too far off in the future, when most will accept transwomen as women.
