Depending on the store you can give a false name with no contact number on a CHATR account. I got a nubia flip top phone for around $80.0 and a year unlimited texts and calls for $149
My 7-11 died so I too went this route. Easy, straight forward, they set it up, gave an address and name and an email. Seems to work but damn that button pressing on the old style phones. But likely lost $50 from the speakout account.

Koodoo Prepaid ($100 for 360 days, only voice and text no data) does not require ID. Process is same as Speakout.

Buy simcard for $10 at Koodoo mobility reseller, then buy prepaid vouchers in denominations of up to $50 at Shoppers Drug Mart, Shell, ESSO's Circle K, etc.. Self activate online here. Self activation accepts multiple vouchers so at minimum you'll need to by 2x $50 vouchers. Places selling vouchers like Rexall, Shoppers Drug Mart, Shell, etc... don't sell sim cards.

Notes:
  • Some mobile phone places won't sell you a sim card and will insist they activate for you. They are misinformed, go to another reseller that will sell simcard only. If you let reseller activate then sim card is free so save $10.
  • The place that sold me simcard wouldn't sell me top up voucher, so bought voucher separately else where.
  • Instant Top Up vs Vouchers. Shell, Rexall pharmacy, etc.. sell the vouchers of the type that the receipt has a PIN number for you to manually input into phone or via online account. Ask before buying. Don't want the Instant Top Up type in which you provide your phone number to the retailer/cashier
  • Places that sell Instant top ups or Vouchers

They asked me if I already have a phone, and I do have an older iPhone 10 sitting on the shelf. They confirmed that their LTE network works on the iPhone 10, so just a SIM card. They also offered to do everything over the phone with an e-sim for my current new iPhone which has no sim tray, but two sim "e-slots". I thought about it, and decided to go for privacy over convenience and stick with a separate sim card with an older phone....assuming I go this route.

I like that they have several "bricks and mortar" boutiques in the GTA....they actually answered the phone when I got the above details, but I'll go into their shop when I'm ready to buy to confirm everything. You have dig into their plans because they lead with the $149 annual plan, but they do appear to offer a one year $100 plan.

Just confirming that I got a new phone number/SIM card for $100/year today (plus $10 one time charge for the physical SIM card) without ID or credit card. PM me if you are interested in the details.
