For the most part they get a work permit (visa) or a green card. Most just get a temporary work permit as that's not too hard to do. Getting a green card is tough and if a girl wants one bad enough she's probably marrying some US citizen to get one. I believe they get in under some kind of "artist" criteria that probably wasn't meant for porno but it allows you temporary access for business (e.g., sort of like Justin Bieber). If you're shooting for one of the established companies you need to have your papers (i.e., they're established companies for a reason and don't want Immigration coming down on them for hiring foreign workers).

As an aside, there's plenty of young, willing, and often stupid American girls so the studios aren't typically bringing in the Canadian equivalent. Usually they've found some super hot Canadian girl who will do triple DP and they want her bad so they help with the paperwork.
