He is intellectually and morally incapable of distinguishing between the two.
That's on him, and you can't reason him out of it. Even if you could get a clear list of his moral priorities (he did say "Medicare for All, wealth inequality, and ending the wars" but it is difficult to know if that list is real or just expedient rhetoric for the moment) you would be up against the fact that he appears unable to distinguish degree.
